NDIS Support Co-ordination assists participants to find the right services and supports to meet their individual needs and assist in achieving their goals.

What you will be doing

• Provide coordination of supports to NDIS participants and assist them in achieving the goals set out in their NDIS plans
• Assist participants to find services, both funded and mainstream to participate more fully in the community
• Assist with building informal and formal supports, resolve issues
• Assist participants to strengthen their own abilities to coordinate and implement supports

Who we are looking for:

• You’ll have a tertiary qualification in Social Work, Psychology, Human Services OR a Certificate IV/Diploma level qualification and significant experience
• Have demonstrated skills and experience in working with children, young people and adults, who meet the eligibility requirements of NDIA
• Intermediate ability to use computer IT systems including MS Word and Excel
• Valid working rights in Australia
• Have a Queensland Provisional Green or above Driver License
• Have a NDIS Yellow Card and Working with Children Check (Blue Card) and any other legislatively required personal history screening as required.
