We are currently seeking a Support and Advocacy Worker to be part of Micah Projects’ Pathways Hospital Admission and Discharge Service which operates within the Home for Good Coordinated Access and Referral Team.
The objective of the service is to work with highly vulnerable people upon discharge from the Royal Brisbane and Women’s Hospital and Princess Alexandra Hospital. Support is provided through follow-up clinical nursing, non-clinical care and co-ordination services to create access to housing, primary and community-based healthcare and social support. This position has been funded by Metro North Hospital and Health Service to expand the reach into and response across the Metro North geographical area.
This position is fixed term, full time until 30th June 2018 with a minimum of 38 hours of work available per week, working Tuesday to Saturday, and is classified under the SCHADS Award at a Level 4.
